You can book with us by following ways:
The method of payment accepted is a cheque payable to Shoal Divers.
Download the Shoal Divers and The London Underwater Centre booking
form in Microsoft Word and the medical
form which is an Adobe PDF document.
Please note, bookings can only be confirmed on receipt of:
- Completed and signed Booking form.
- Completed and signed self Medical form. (You do not need to go to a doctor as long as there are no YES answers
to the medical questionnaire on the form.)
- Full payment received prior to commencement of the course.
